Circumcision is a surgery in which the foreskin of the   penis   is removed. It is done for various reasons. They are narrowing of the skin of the penis where it cannot be retracted. This condition is called phimosis. Another reason why this surgery is done is for repeated infection of the foreskin. This condition is called balanoposthitis and is commonly seen in patients with diabetes. However, the most common indication for circumcision is religious as in Muslims and Jews, and cultural as is followed in North America. Other less common reasons to do circumcision are cancer involving the foreskin and viral warts of the foreskin. How is circumcision done? Acute pyelonephritis (APN) is a technical term for infection of the kidney. It is a part of the spectrum of urinary tract infection or UTI . UTI can occur anywhere from kidney, ureter, urinary bladder, prostate and urethra. Symptoms of APN The individual complains of pain abdomen, fever and chills. However there are many who do not suffer from all these symptoms and may only have some of them. Fatigue, generalized weakness and vomiting also are common. They may not have symptoms like burning urination or painful urination. APN usually is seen in middle aged and elderly people and less in young people. It is rarely seen in children. Causes APN is almost always caused by infections by common urinary infection causing bacteria like E. coli, Proteus, Enterobacter and Pseudomonas. This is a condition where there is inability to retract the skin of the penis from the head of the penis. All male children have this condition when they are born. This is called physiological phimosis. As they grow up, the skin gradually becomes detached from the head of penis and can be retracted. In 90% of the children this happens by the age of 3 years. However, in some children the separation may happen as late as at the age of 17 years. Phimosis is called pathological when the skin becomes unretractable as a result of tear or injury to the foreskin. Symptoms of phimosis: The child with phimosis may present with pain while urinating, recurrent urinary tract infection, ballooning of prepuce while urinating or itching of the tip of penis. An adult may, in addition present with pain in penis on erection or intercourse as the skin is adherent to the glans. Phimosis may also be seen in people with diabetes and infections of the foreskin.
